    Care Manager Hourly

    Position Summary:

    A Care Manager is responsible for coordinating care and support services for clients, ensuring they receive the necessary resources to improve their quality of life.

    As an integrated health organization Acenda values the use of evidence-based practice or EBP's.

    $500 Sign-On Bonus Available

    Duties and Responsibilities:

    • Care Coordination: Oversee and manage client care plans, ensuring timely access to services and resources.
    • Collaboration: Work closely with providers, clients, and families, to develop and implement comprehensive care plans tailored to individual needs.
    • Monitoring Progress: Regularly assess and monitor client progress, addressing any barriers to treatment and adjusting care plans as necessary.
    • Client Advocacy: Advocate for clients' needs, ensuring they receive the highest quality of care throughout their medical journey.
    • Documentation: Maintain accurate and up-to-date case documentation and client records in compliance with regulatory standards.
    • Adheres to a flexible schedule based on client and program needs.
    • Demonstrates competent written and verbal communication with referral sources and external customers. 
    • Attentive and responsive to customer needs in a helpful, supportive and timely manner. 
    • Works collaboratively with coworkers and agency staff to effectively communicate and problem-solve. 
    • Meets obligations of the program (contractual, licensing, Medicaid, etc.). 
    • Follows program policies and procedures and encourages the same in others. 
    • Educates self on issues impacting the program population. 
    • Operates conscientiously in accordance with program budget (i.e., use of client engaging funds, office supplies). 
    • Performs other related duties as assigned.

        • Establishes rapport, gathers clinical data, assesses readiness for treatment; and applies criteria for diagnosis
        • Screens for psychoactive substance toxicity, intoxication, withdrawal symptoms, co-occurring mental health conditions, and risk of harm to self or others.
        • Interprets screening results to determine treatment priorities
      • Children and Youth Programs
        • Demonstrates knowledge of age-appropriate developmental milestones.
        • Formulates measurable, individualized treatment goals and objectives.
      • Housing Support Services for Homeless Clients
        • Applies evidence-based or accepted case management practices.
        • Demonstrates communication and advocacy skills.
        • Understands and complies with Fair Housing rules and regulations.

    Education, Work Experience and/or Skills Requirements:

    • Bachelor's/Master's degree (dependent on program) in psychology, social work or a related filed, plus 2 years of relevant experience.
    • Must have and maintain a valid driver's license with an acceptable driving record
    • Must be able to exercise independent thinking and good judgment under all circumstances.
    • Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.

    Physical Requirements:

    • While performing the duties of this job, the employee is occasionally required to remain in a stationary position, often standing or sitting for prolonged periods.
    • Repeated motions that may include the wrists, hands and/or fingers
    • The employee must occasionally move about to accomplish tasks, adjust or move objects up to 20 pounds.
    • Must be able to communicate with others to exchange information
    • Ability to drive a vehicle

These events include Concept Development, Doctrine Development and Analysis. Participants: Navy/Maritime Military, Joint Military, Coalition Nations, and Civil Agencies. Primary focus areas have been Command and Control, Ballistic Missile Defense, Integrated Air and Missile Defense, Strike Warfare, Unmanned air, undersea and surface vehicles, Distributed Netted Systems, Anti-Submarine Warfare, Large Area/Global Concept of Operations, Maritime Domain Awareness, Global Maritime Awareness, Red Team, Health Services Support, Mine Warfare, Joint/Navy Command Organization and Relationships, and the technologies, systems and processes involved with the above.

     CONTRACTS

    Current: Supporting contracts at Navy Warfare Development Command (Experimentation and Analysis) in Norfolk VA.
