A health services organization in New York City is seeking a Case Manager to join their team in Brooklyn. This role supports care coordination for clients with complex medical and behavioral health needs through field-based engagement, documentation, and collaboration with care teams and community partners.
About the Opportunity:
- Shift: Day
- Schedule: Monday to Friday
- Hours: 9am to 5pm
- Setting: Field-based and on-site
Responsibilities:
- Coordinate care for assigned clients across adult and youth programs, ensuring services align with each client's needs and plan of care
- Conduct field visits in homes, schools, medical settings, and community locations as needed
- Document client activity, outreach, progress, and care coordination details in electronic records and related systems
- Schedule appointments, follow up on medical needs, and communicate with families, providers, and interdisciplinary team members
- Support treatment planning, service referrals, benefits navigation, and ongoing case management for a defined caseload
Qualifications:
- At least 1 year of experience in healthcare, social work, case management, discharge planning, or a related service coordination setting
- Bachelor's degree in a health, human services, or related field
- Strong clinical, administrative, organizational, written, and verbal communication skills
- Proficiency with Microsoft Word and Excel
- Ability to maintain accurate documentation and manage a caseload independently
- Fingerprint clearance and SCR clearance required
Desired Skills:
- MSW degree
- Bilingual skills, especially Spanish or Creole
- Experience with ACS-related work
- Experience supporting children, adolescents, or adults with mental health needs
- Experience with field-based case management
- Experience with care navigation or health home services
- Familiarity with EPIC, RMA, NextGen, or Connections
