    Job Description:

    We are seeking dedicated and compassionate Case Managers to join our team in the Burbank area. In this role, you will work directly with adults with intellectual disabilities, providing support and connecting them with resources to help them thrive in their communities. This position requires travel to clients homes within the designated area.

    Key Responsibilities:

    • Provide case management services to adults with intellectual disabilities.

    • Conduct regular home visits to assess client needs and monitor progress.

    • Develop, implement, and monitor individualized service plans.

    • Advocate for clients and coordinate with families, service providers, and community resources.

    • Maintain accurate and timely documentation and case notes.

    • Participate in team meetings and trainings as required.

    • Work occasional weekends as needed.

    Work Schedule:

    • Monday to Friday, 8:30 AM 5:00 PM

    • Occasional weekend work as required

    Compensation & Benefits:

    • Pay: $26.00 $29.00 per hour, based on experience

    • Mileage reimbursement for travel

    Requirements:

    This role is crucial in supporting our clients with developmental disabilities, so we are specifically seeking candidates who meet all qualifications.

    Currently, we are looking for individuals who are fluent Spanish speakers able to read, write, and speak Spanish along with knowledge of the Regional Centers, extensive case management experience, and experience working with government agencies on behalf of disabled clients.

    Must be bilingual in English and Spanish (read and write) 

    • Valid drivers license and reliable transportation.

    • Extensive Case Management experience
    • Knowledge of regional centers 
    • Ability to work independently and manage time effectively.

    • Experience working with individuals with intellectual disabilities preferred.
