Case Manager

The following statements are intended to describe the general nature and level of work being performed. They are not to be construed as an exhaustive list of all duties and responsibilities required of personnel so classified. Position Summary: A Mental Health Worker/Case Manager will assist in the development of treatment plan(s) that are appropriate to the needs and desires of the client. The treatment plan(s) will aim to promote recovery and stability for community living and helping the client to achieve higher levels of function. Core Duties And Responsibilities: Monitor client compliance with treatment as prescribed by treatment team- this may or may not be done in the office or by means of field services.Develop individualized case management treatment plan(s) with the clients in the Full Service Partnership (FSP) Program.Assist with establishing coordination with local agencies in the provision of services.Assist with the development and maintain resource directory for agency.Update and maintain client records.Arrange for medical care-exams, emergency shelter, food and/or education services for clients at large.Facilitate support/self-help groups for clients, family members and the community at large.Participate in case disposition meetings on a routine basis with the assigned treatment team.Provide and document services in accordance with the Rehabilitation Option/Targeted Case Management requirements.Participate in the Mental Health education program provided to the community.Coordinate client care with other providers both within and outside the agency.Attend and participate in all in-service training as necessary.Responsible for timely completion of documentation in the clients clinical record per Enki, LA County Department of Mental Health and legal requirements.Responsible for meeting assigned minimum productivity requirements on a monthly basis.Other duties as assigned.Comply with all Company policies and procedures as applicable.
