Case Manager - Forensic

Adapt Integrated Health Care is looking for a qualified Case Manager to join their Forensic Mental Health team!

The Forensic Case Manager assists in the development and maintenance of a healthy, supportive environment to make provision of effective therapeutic interventions possible within the community. The Case Manager works with individuals diagnosed with mental illness who are involved in the criminal justice system to promote recovery, independence and successful community living, by or through (a) Communication of hope, and promotion of emotional, behavioral and psychological growth through persistent efforts to attain individual goals; (b) The promotion of skills and knowledge to help individuals effectively manage their mental health concerns and develop a sense of hope and a sense of self that is not illness dominated; and (c) Provision of a humane service environment that affords reasonable protection from harm including re-traumatization.

Provides case management, skills training and mentoring services to individuals with mental illness within the community and outside of Adapt Integrated Health Care offices. Provision of treatment services are informed by the Individual Service and Support Plan (ISSP) and clinically supervised by the primary clinician.

Services are provided within a program for the Mental Health Division of the Health and Social Services Department in consultation with the treatment team. This classification provides advocacy and linkage in planning, monitoring, coordination, and implementation of the treatment interventions as identified in the ISSP. In addition, Case Manager's assist individuals in problem-solving to reduce barriers to recovery and support the development of resources within the community while coordinating services with other community and treatment providers.

Case Manager's provide a variety of treatment services including individual and group skills training and development; case management services; coordination and monitoring daily structured activities of clients; performs crisis intervention as necessary under the guidance of the Forensic Mental Health Program Coordinator (QMHP) and/or supervisor; provides consultation and training; facilitates communication and correspondence with other Adapt Integrated Health Care staff, community partners, other treatment providers, peers, and all others as needed. This classification coordinates and collaborates with a multi-disciplinary treatment team and is responsible for work performed in assigned specialized program area.

Qualifications

• Bachelor's degree and two years' experience on a social service and/or mental health setting or a satisfactory equivalent combination of education, experience, and training.
• Must be 21+ and possess a valid Driver's License.
• Must be eligible to register as a QMHA through MHACBO.

Primary Responsibilities

.370 Project (Aid and Assist)

• Assist in housing and stabilization services.
• Assist with diversion from State Hospital for clients deemed unable to Aid and Assist by implementing a community plan for restoration
• Liaison with criminal justice partners and State Hospital to reduce length of stay for clients who are there for restoration
• Provides case management, individual and group skills training and development, mentoring and crisis intervention as directed by a QMHP and documented in the Individual Service and Support Plan (ISSP); uses interventions informed by Evidence-based Practices endorsed by Adapt Integrated Health Care.
• Actively promotes the harmonious delivery of services through coordination of interventions in collaboration with treatment team members and foster providers achieved by ongoing communication and active problem solving with the treatment team and foster providers about outcomes, barriers and case observations.
• Maintains and completes required records, reports, documentation as required by departmental policy and state and federal law.
• As many of the services are provided with in the community and throughout Douglas County, driving is an essential and daily function of this position; transports individuals, family members and other supports to/from services
• Participates in professional development through individual and group supervision, unit and agency staff meetings, workshops and Essential Learning tutorials; develops, modifies, and or obtains training programs and materials in consultation with supervisor and program manager; promotes peer professional development by sharing training materials and or training others in successful interventions.
• Unique duties maybe assigned to staff members by their Program Manager or Supervisor, to meet agency needs; current incumbent in this position supports 370 program clients and works with law enforcement, the State Hospital and case managers to coordinate their re-entry into communities.

PSRB (Psychiatric Security Review Board)

• Provide PSRB Supervision
• Track client appointments and compliance.
• Coordinate cares with therapist and community treatment staff.
• Search client's house twice a month as mandated by PSRB once unannounced and once announced.
• Follow and act according ODOC rules and regulations regarding the care and treatment of prisoners.
• Report compliance to PSRB.

Jail Diversion Program

• Provide intensive case management services.
• Provide mobile crisis services as necessary.
• Work flexible hours to engage clients on release from jail and during crises.
• Liaise with local law enforcement, parole and probation, and jail staff.
• Be able to attend appointments in the community and meet the client "where they are". This includes jail, the homeless shelters, homeless camps, streets, under bridges etc.
