Program: Forensic Intensive Recovery (FIR)

Program Overview:
Forensic Intensive Recovery (FIR) is a prison deferral initiative that offers substance use treatment as an alternative to incarceration. PHMC's FIR team provides clinical evaluation, case management, and peer support to participants.
Job Overview:
The FIR Case Manager works directly with participants who are court-stipulated for treatment services with oversight from the Adult Probation and Parole departments. The Case Manager's primary responsibility is to ensure services are provided to participants and to document those services according to DAP, CBH, Forensic Services, and PHMC requirements. The Case Manager supports participants while working with all levels of management within the treatment program, Forensic Services, and the recovery community.

Responsibilities:
- Provide case management services to a chemically dependent forensic population.
- Establish and maintain service productivity goals at the minimum designated service units per week.
- Document service provision within 24 hours.
- Participate in monthly team meetings and scheduled case conferencing with Probation, meeting reporting requirements.
- Participate in biweekly supervision meetings.
- Collaborate and attend meetings with Probation/Parole, treatment providers, and other collaborating agencies.
- Complete discharge summaries within timeframes specified by the program.
- Assist individuals with links to recovery support services, including PRO-ACT recovery center, Access to Recovery, faith-based institutions, mental health services, and other identified community supports.
- Create weekly schedules detailing activities to be completed for each week.
- Maintain participant records in accordance with agency fee-for-service standards, managed care, and licensing requirements.
- Prioritize helping participants enroll in treatment, physical health services, DPA, vocational services, and educational services.
- Identify, coordinate, and link community resources to each participant to promote accessibility to recovery-oriented community services.
- Coordinate and communicate with other programs within and outside the agency to foster a collaborative team approach.
- Spend an average of 80% of each workweek in the field for off-site participant appointments and meetings.

Qualifications:
- Bachelor's degree required, including at least 18 credits in psychology, social work, sociology, criminal justice, or a related social science field
- Minimum of 1 year of experience performing counseling or case management work in a substance use treatment setting
- Valid driver's license required
