Case Manager LPN
Setting: Healthcare Facilities
Job Type: Contract - Full Time
Pay: $50 / Hour
Hours: 40.00
Location: Springfield, MA
The LPN/LVN Case Manager is responsible for coordinating
continuum of care activities for assigned patients and ensuring optimum
utilization of resources, service delivery, and compliance with medical regime.
Minimum Requirements:
- Current LPN/LVN
licensure in state practicing
- At least one year of Case Management experience preferred
- Complies with all relevant professional standards of
practice
- Current CPR if applicable
- TB questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or state
regulation)
- Must meet all federal, state and local requirements
- Must be at least 18 years of age
