Division Overview: The Housing Division provides safe, affordable housing paired with wraparound supportive services to individuals and families who have experienced chronic homelessness, mental illness, substance use disorders, or other significant barriers to stability. Guided by Housing First principles, the division works to promote long-term housing stability, improve health and well-being, and foster independence by integrating case management, clinical support, and community engagement. Through partnerships with public agencies, funders, and community organizations, the division advances the nonprofits mission to reduce homelessness and create equitable pathways to housing security and self-sufficiency.
We are currently looking for full-time Case Manager
Starting at $44,676 - $47,650; contingent upon experience, education, etc.
Position Summary: Through the Housing Is Recovery program, TASC assists individuals who have substance use disorders or serious mental illness in securing and maintaining stable, affordable housing. Participants receive TASCs Specialized Case Management and access to a network of community services that promote recovery, stability, and independence. By combining safe housing with personalized support, TASC helps individuals build the skills and connections needed to thrive in their communities.
The Case Manager provides specialized case management and care coordination in securing stable housing and accessing essential services. The Case Manager conducts comprehensive housing screenings and assessments of social determinants of health to inform individualized stabilization plans for immediate and long-term goals. The Case Manager works closely with clients to navigate complex service systems and reduce barriers to accessing housing stability. The Case Manager also facilitates timely referrals to community-based providers, behavioral health services, and support programs based on each clients assessed level of care. All services are delivered with a trauma-informed, person centered approach that honors the clients personal readiness, and willingness to engage in care and support.
