Case Manager

We are seeking a skilled professional to coordinate patient care through acute care hospitalization, focusing on discharge planning and transition processes.

Key Responsibilities

• Coordinate patients through acute care hospitalization.
• Manage all aspects of discharge planning and transition processes.
• Perform utilization management and quality screenings.
• In-Patient Case Management is required.

Required Skills and Experience

• Minimum 2 years of acute care experience strongly preferred.
• Experience in Case Management and Managed Care.
• Hospital experience required, including discharge planning and placement in rehabilitation facilities.
• Experience with subacute patients required.

Schedule

• 8:00 AM – 4:00 PM.
• Rotating weekends: Every 4th or 5th weekend.
• Holidays: One summer and one winter holiday.
