Case Manager

Join our dynamic team in Morristown, NJ, where you will play a crucial role in coordinating patient care and ensuring smooth transitions from acute care hospitalization. We are looking for an experienced professional to manage all aspects of discharge planning and transition processes.

Duties

• Coordinate patients through the acute care hospitalization process.
• Manage all aspects of discharge planning and the transition process.
• Perform utilization management and quality screenings.
• In-Patient Case Management is required.

Skills and Experience

• 2 years of acute care experience strongly preferred.
• Experience in Case Management and Managed Care.
• In-Patient Case Management experience is required.
• Hospital experience required, including discharge planning and placement in rehabilitation facilities.
• Minimum 2 years of experience with acute inpatient hospital discharge planning.
• Experience with subacute patients required.

Schedule

• Schedule: 8:00 AM – 4:00 PM
• Rotating weekends: Every 4th or 5th weekend
• Holidays: One summer and one winter holiday
