Job Title: Case Manager
Location: Morristown, NJ
Duration: 13 Weeks 
Shift: 8:00 AM – 4:00 PM
Rotating weekends: Every 4th or 5th weekend.
Holidays: One summer and one winter holiday.

Pay Rate: $57/HR ON W2

Summary:

The Case Manager coordinates patients through the acute care hospitalization and manages all aspects of discharge planning and transition process.
The role also performs utilization management and quality screenings, with a focus on in-patient case management.

Responsibilities:

• Coordinate patients through the acute care hospitalization.
• Manage all aspects of discharge planning and transition process.
• Perform utilization management and quality screenings.
• In-patient case management is required.

Requirements:

• BSN Required. 

• NJ or Compact state RN license.

Required Skills:

• 2 years of acute care experience strongly preferred.

• Experience in case management and managed care.

• In-patient case management is required.

• Hospital experience, including discharge planning and placement in rehabilitation facilities.

• Minimum 2 years of experience with acute inpatient hospital discharge planning.

• Experience with subacute patients required.
