We are seeking a skilled professional to coordinate patient care during acute care hospitalization and manage all aspects of the discharge planning and transition process.
Duties
- Coordinate patients through the acute care hospitalization process.
- Manage all aspects of discharge planning and transition processes.
- Perform utilization management and quality screenings.
- In-Patient Case Management is required.
Skills
- 2 years of acute care experience strongly preferred.
- Experience in Case Management and Managed Care.
- In-Patient Case Management is required.
- Experience with Sub-acute patients is a must.
Schedule Notes
- 8 AM - 4 PM
- Rotating weekends every 4th or 5th week.
- 1 summer and 1 winter holiday required.
