Join a Mission That Changes Lives
At the Crisis Center of Comal County (CCCC), our mission is to embrace survivors of crisis and trauma and empower them to positively change the trajectory of their lives.
Every day, survivors take courageous steps toward a safer and more stable future. As a Case Manager, you'll walk alongside them through that journey—helping individuals and families navigate immediate challenges, identify their strengths, overcome barriers, secure permanent housing, and build the skills and confidence needed for lasting independence.
This isn't simply a job helping people access resources. It's an opportunity to build relationships, create possibilities, celebrate progress, and help survivors recognize that a different future is possible.
If you're compassionate, organized, solution-focused, and passionate about empowering people through trauma-informed support, we'd love to have you on our team.
About the Role
As a Case Manager, you'll provide comprehensive, trauma-informed case management to individuals and families living in our emergency shelter as well as clients who have transitioned into permanent housing.
You'll get to know each client as an individual, assess their needs and goals, develop personalized service plans, coordinate resources, advocate alongside them, and provide support as they work toward safety, housing stability, and long-term independence.
You'll work both inside and outside the shelter—meeting with clients, conducting home and community visits, coordinating services, building relationships with community partners, and helping clients navigate the challenges that can arise after moving into permanent housing.
This position is ideal for someone who builds trust easily, thinks creatively, stays calm during crisis, and believes that empowerment means walking alongside clients rather than making decisions for them.
What You'll Do
- Provide trauma-informed case management, advocacy, crisis intervention, safety planning, and support to residential shelter clients and clients who have transitioned into permanent housing.
- Conduct comprehensive assessments and develop individualized service plans focused on safety, housing stability, employment, education, financial wellness, health, and long-term independence.
- Monitor client progress, evaluate outcomes, and adapt service plans as client needs and circumstances change.
- Support shelter intake, orientation, and client transitions into shelter while helping create a warm and supportive experience.
- Provide ongoing crisis intervention, emotional support, and problem-solving for individuals and families experiencing domestic violence, sexual assault, homelessness, and other crises.
- Conduct individual case management meetings, support groups, and life-skills classes that help clients build skills and work toward their goals.
- Coordinate and provide transportation for clients to appointments, housing, employment, and other essential services, including safely operating agency vehicles.
- Lead house meetings and assist with implementing shelter expectations and daily routines.
- Collaborate closely with Case Aides and other shelter staff to support client goals and maintain a safe, welcoming shelter environment.
- Identify clients who may benefit from continued support after leaving shelter and provide ongoing post-housing case management.
- Conduct home visits, office visits, community visits, and virtual meetings to monitor housing stability and client progress.
- Help clients develop skills related to budgeting, employment, transportation, education, parenting, healthcare, financial management, and independent living.
- Connect clients with community resources, including housing programs, employment services, healthcare and behavioral health providers, legal assistance, childcare, food assistance, and financial literacy programs.
- Advocate with and alongside clients as they access benefits, public assistance, disability services, housing resources, and other supports.
- Build collaborative relationships with landlords, housing providers, schools, employers, healthcare providers, community agencies, and other service partners.
- Maintain timely and accurate case documentation, electronic records, assessments, referrals, service plans, and grant-related documentation.
- Assist with program data collection and grant reporting requirements.
- Participate in staff meetings, case reviews, multidisciplinary meetings, trainings, and professional development opportunities.
- Participate in the agency's Sexual Assault Crisis Response on-call rotation.
- Represent CCCC professionally in the community and support outreach, fundraising activities, and special events as assigned.
Who You Are
- You believe every survivor deserves safety, dignity, compassion, and the opportunity to create a future of their own choosing.
- You are an excellent listener who can build genuine relationships while maintaining healthy professional boundaries.
- You understand that effective case management is about empowering clients—not doing things for them that they can do for themselves.
- You are compassionate while also being organized, accountable, and able to maintain appropriate boundaries.
- You remain calm and thoughtful when working with clients experiencing crisis, housing instability, financial hardship, family conflict, or other significant challenges.
- You are a creative problem-solver who looks for possibilities and resources when the path forward isn't obvious.
- You can balance immediate client needs with long-term goals and planning.
- You are comfortable advocating for clients while respecting their autonomy and supporting them in making their own decisions.
- You communicate effectively with clients, coworkers, supervisors, and community partners.
- You are organized and able to manage multiple clients, deadlines, appointments, documentation requirements, and competing priorities.
- You work well independently while also valuing collaboration with a multidisciplinary team.
- You are open to feedback, committed to continuous learning, and willing to adapt as client and agency needs change.
