Believe In Me Empowerment in New Haven, CT is looking for a passionate and dedicated Case Manager to join our mission-driven team. Responsibilities:

• Conduct comprehensive client assessments to identify needs, strengths, and barriers to stability
• Develop and implement individualized service plans in collaboration with clients
• Connect clients with community resources, social services, and support programs
• Monitor client progress and adjust care plans as needed to ensure goal achievement
• Maintain accurate and up-to-date case documentation and client records
• Advocate on behalf of clients with external agencies, providers, and stakeholders
• Facilitate referrals to mental health, housing, employment, and other support services

Requirements:

• Bachelor's degree in Social Work, Human Services, Psychology, or a related field
• Prior experience in case management, social services, or community outreach preferred
• Strong knowledge of local New Haven and Connecticut community resources
• Excellent communication, advocacy, and interpersonal skills
• Ability to manage a caseload effectively while maintaining attention to detail
• Empathetic, culturally competent, and trauma-informed approach to client care
• Proficiency in case management software and Microsoft Office Suite
