Position Overview:

Founded in 2001, the NW Instituto Latino (NWIL), now El Jardín, provides low-barrier, no-cost culturally and linguistically specific recovery support and harm reduction services for the Latinx community in the Portland metro area.

The El Jardín Case Manager will assist with referrals to community services for mental health, SUD treatments, recovery-based support, education, and supportive programs. This program will also help with common community reentry resources and assistance for clients and their families. This program will offer culturally diverse and bilingual assistance.

The Case Manager must share our passion for improving people's lives by providing access to the best outcome possible based on the person's strengths. From day one, you will have a hands-on impact, facilitating client services with the underlying objective of enhancing the quality of outcomes and client satisfaction. The role demands exceptional active listening and decision-making skills. You will be expected to deliver a level of care, comfort, and safety that exceeds industry standards.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Coordinate and facilitate clients' needs through assessment, evaluation, planning, and implementation.
  • Communicate clients' needs to various care team members and follow up accordingly.
  • In collaboration with the client and community partners, develop and manage case management and recovery support plans.
  • Work collaboratively with clients, families, mentors, and community partners to ensure high-quality services.
  • Act as the Client's advocate as it relates to interdisciplinary healthcare, housing, the Department of Criminal Justice, and the Department of Human Services.
  • Maintain comprehensive case manager records through detailed documentation.
  • Coordinate with third-party support regularly.
  • Manage case assignments, craft service plans, track progress, and execute case closure.
  • Assess and respond accordingly to treatment barriers and psychosocial issues.
  • Responsible for adhering to generally accepted employment attendance policies.
  • Responsible for understanding and following all safety rules and standards in compliance with the company policies.
  • Attends El Jardín professional development training and staff meetings.
  • Assessing clients' physical and mental wellness, needs, preferences, and abilities, using these to develop tailored care plans.
  • Working with clients, family members, friends, support networks, and health professionals to put care plans in place.
  • Listening to clients' concerns and providing counseling or intervention as required.
  • Recording clients' progress, including charting referrals, home visits, and other notable interactions.
  • Evaluating clients' progress periodically and making adjustments as needed to improve clients' outcomes.
  • Following up with discharged clients to ensure they are satisfied with services and still in good physical and mental health.
  • Support the needs of the Community Center as needed
  • Completes other duties as assigned by the Program Manager.

MINIMUM QUALIFICATIONS AND EXPERIENCE

  • Bilingual Spanish/ English required
  • Bicultural- Latinx Communities required
  • Pre-employment and reasonable-suspicion drug testing may be required.
  • This position typically requires a High School Diploma/GED, plus 2 years of peer support experience and 1 year of peer supervision experience.
  • Must have reliable transportation and maintain a valid driver's license for the state of residency, valid vehicle registration, and a driving record permitting coverage under the corporate liability policy. If driving a personal vehicle for work purposes must also have and maintain personal auto liability coverage. All employees driving for work, regardless of whether they are driving a personal or agency vehicle, must complete the agency's driving approval process before driving for work.
  • Experience in outreach and community partner networking.
  • Understanding and support of Harm Reduction Approaches and multiple pathways to recovery.
  • QMHA, THW, or PWS preferred.
  • At least 3 years of direct service experience with Latinx populations.
  • Demonstrated success at working effectively in a diverse, collaborative team environment, with skills in promoting inclusion and cultural responsiveness.
  • Knowledge of community resources and referral processes for basic needs, crisis intervention, recovery support, employment, and medical and mental health.
  • Experience working with minority communities and knowledge of the current disparities that certain communities of service users may face more broadly in society is required.
  • Ability to work independently and as a team player.
  • Able to establish and maintain professional boundaries while providing supportive services and building trusting relationships with clients, staff, and community partners.
  • Excellent communication, trauma-informed approach, and conflict-resolution skills.
  • Well-developed documentation skills and a strong commitment to quality management; ability to write accurate and concise reports while meeting tight deadlines.
  • Computer literate; functional ability with Microsoft programs and/ or Google Workspace.
  • Able to articulate information clearly to people of a variety of cultural, linguistic, and educational backgrounds.
  • Able to problem-solve and develop creative approaches.
