A nonprofit organization in Queens, New York is seeking a Case Manager to provide supportive services for formerly homeless individuals in permanent housing.
About the Opportunity:
- Shift: Day shift
- Schedule: Monday to Friday (with one late night)
- Hours: 9am to 5pm (with one 11am to :pm)
- Setting: On-site
Responsibilities:
- Provide direct case management services to a caseload of approximately 15 clients
- Conduct home visits, assessments, psychosocial evaluations, and crisis interventions as needed
- Assist clients with benefits, entitlements, identification, recertification paperwork, and other housing-related documentation
- Connect clients to psychiatric, medical, substance use, job training, food, recreational, and community support services
- Maintain accurate case records and coordinate ongoing support plans to help clients remain housed and progress toward recovery goals
Qualifications:
- At least 2 years of experience working with homeless adults and substance use issues
- Bachelor's degree
- Experience with harm reduction and motivational interviewing techniques
- Excellent computer skills, including proficiency in Microsoft Word and Excel
- Excellent writing and communication skills
- Experience working with formerly homeless individuals, individuals in shelter settings, or clients with mental health needs
Desired Skills:
- Bilingual Spanish/English
- Experience with NYC government benefits systems
- Background in social services, social work, or mental health
