Case Manager (RN)

Position Summary:

Manages a designated caseload, to coordinate and complete timely assessments, planning, implementation and evaluation of discharge plans and care transitions across the continuum of care. Collaborates with medical staff, unit managers, and all professional care giver disciplines to plan and coordinate the plan of care with patients and families throughout the acute continuum of care and into post acute settings. Ensures optimum utilization of the patient's and the Health System's resources and perform these duties within the requirements of CMS and other external review agencies. Responds to Director/Manager as required.

Position Qualification:

Experienced case manager in acute care: RN w/TN license or Social Work degree

Strong organizational and multitasking skills

Excellent communication and interpersonal skills

Ability to assess patient needs and develop care plans

Knowledge of health care systems and resources

Understanding of medical terminology and documentation

Prefer Cerner experience
