Case Manager RN

Assumes accountability for assessing, coordinating implementing care service from admission through discharge for assigned patients.Coordinates communication of all patient related information to appropriate personnel internally externally. Adheres to agency standards ensures confidentiality of patient information patient rights. Requirements: RN License in the State of Rhode Island or a state participating in the Nursing Compact BSN preferred. For AD graduates applying, must provide evidence of enrollment for BSN program with completion within 3 years Basic Life Support certification from the American Heart Association One year acute care nursing experience One year home care experience Valid drivers license auto liability insurance as mandated by State of RI
