Your future role at a glance:

Location: Jacksonville, FL 

Facility: Ascension St. Vincent's Southside Hospital

Schedule: Day Shift | Full-time

How you'll make an impact in this role:
  • Orchestrate the patient journey by leading interdisciplinary teams to design and execute seamless care plans from admission through discharge.
  • Champion patient-centered care by integrating family values and personal preferences into every stage of the clinical process.
  • Drive clinical outcomes through proactive assessments and continuous monitoring to ensure every intervention is effective and purposeful.
  • Eliminate barriers to recovery by advocating for essential resources and resolving obstacles that impede high-quality care.
  • Serve as a strategic lead on medical documentation and post-discharge planning to ensure long-term patient success and compliance.
What minimum qualifications you'll need:

Licensure / Certification / Registration:

  • Clinical Counselor preferred.
  • Social Worker specializing in Clinical Practice credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
