As a national leader in veteran services, we are seeking a dedicated Case Manager to provide comprehensive support to veterans residing in our emergency shelter and transitional housing programs in Worcester.
In this role, you will manage a diverse caseload, helping residents navigate the transition from homelessness to stable transitional housing. You will conduct intakes, develop individualized service plans, and connect veterans to vital healthcare, mental health, substance use, and benefits resources. Operating directly at our Worcester facilities, you will provide the intensive stabilization, goal tracking, and advocacy our nation's heroes need to rebuild their lives.
This position is based at our Cambridge St. facility in Worcester, MA.
WHAT YOU'LL BE RESPONSIBLE FOR DAY TO DAY:
- Perform Client Intakes: Conduct individual screenings, evaluations, and comprehensive biopsychosocial assessments for all assigned program participants.
- Identify Housing Barriers: Evaluate each participant's unique skill set and specific obstacles to securing long-term, stable housing.
- Deliver Case Management: Provide direct intervention, supportive service outreach, and individualized care coordination to encourage housing placement readiness.
- Assist with Essential Navigation: Guide participants through employment searches, benefit applications, medical or behavioral health appointments, and transportation coordination.
- Build Supportive Relationships: Establish strong, trust-based connections with veterans and their families to promote the highest level of independent functioning.
- Coordinate Ongoing Services: Meet regularly with assigned participants to provide feedback, monitor progress, and align daily activities with personal goals.
- Maintain Accurate Documentation: Prepare precise case records, progress notes, and individualized service plans while executing ongoing data collection.
- Ensure Audit Readiness: Keep all participants' files compliance-ready while updating key personnel on resident progress and program issues.
WHAT YOU MUST HAVE:
- Minimum of Bachelor's degree in Human Services, Social Work or related field.
- Minimum of three (3) years' experience in program management, case management, substance abuse prevention, outreach, and/or services to the homeless or veteran populations.
- Must have strong computer skills and knowledge of Microsoft Office Suite.

Provide support to previously homeless Veterans through Case Management services in our housing program on Canterbury Street in Worcester, MA Be a part of our team and work with up to 30 clients in developing and achieving treatment goals. Provide weekly, monthly and annual reporting on clients and statistical data. Participate in annual inspections conducted by funding agencies. Administrative and case management skills are necessary for this position.
This position will be based out of our location on Canterbury Street in Worcester, MA. This is a full-time position with full benefits (health, dental, vision, 403B, paid time off, short/long term disability and life insurance, etc.) The annual pay is $42,000 and is non-negotiable.
Must be able to pass a CORI and background investigation. Must be able to start within two weeks of receiving a job offer.

Knowledge, Skills, and Abilities:
- Demonstrated knowledge and experience in working with persons of low income.
- Demonstrated knowledge and experience in working with veterans preferred; must have sensitivity to veterans-related needs and issues.
- Sensitivity to people with low socioeconomic status and minority cultural backgrounds.
- Knowledge of local housing market in service area.
- Knowledge of federal, state and local laws regarding housing and tenancy, highly valued.
- Ability to interpret leases, housing laws and regulations and then communicate them to their clients.
- Ability to network and provide a positive image for the program, provide empathetic support for the clients, while maintaining professional distance and objectivity.
- Strong computer skills to include Microsoft Word, Excel and Outlook.
- Ability to give presentations and speak in front of small to mid-sized groups about Veterans Inc. programs and services.
- Strong conflict negotiation and resolution skills.
- Frequent travel throughout assigned territory required. Must have valid driver's license and own personal, reliable transportation.
- Ability to maintain satisfactory attendance and punctuality.
Minimum Requirements:
- Bachelors in Human Service field with 2 years of experience in counseling, substance abuse prevention, and/or services to the homeless.
- Outstanding communication skills and strong interpersonal skills, including techniques for positive motivation, handling difficult people, and teamwork.
- Knowledge of assigned community and the resources available to persons in need of services.
- Must be able to pass a CORI background records check.
