Case Managers

Case Managers Needed – Bronx & Brooklyn, NY

We are seeking compassionate and experienced Case Managers to support individuals and families in need of social service assistance.

Bachelor’s degree is required.

Schedule: Monday–Friday, 9:00 AM–5:00 PM
Job Type: Full-time
Pay: $26.00–$28.00 per hour
Locations: Bronx and Brooklyn, NY

Responsibilities:
• Interview and evaluate applicants for services
• Develop individualized service plans and goals
• Assist clients with implementing their service plans
• Connect clients with appropriate community resources
• Review documentation to confirm program eligibility, including Medicaid and HIV-status documentation
• Assist clients with intake applications and required forms
• Create, organize, and maintain confidential client files
• Conduct initial client and family assessments and periodic reassessments
• Maintain accurate and timely case documentation
