    Position – Assisted Living/CBRF Facility Administrator 
    Location: Wellington Place at Biron, Wisconsin Rapids, WI (On-site) 
    Status: Full-time

    Job Summary: 
    As a Facility Director at Wellington Place at Brion, you will oversee daily operations, inspire and develop a strong team, and foster a welcoming, resident-centered environment where quality care and exceptional service are top priorities.

    The ideal candidate is a hands-on leader who embraces positive change, builds strong relationships with residents, families, and staff, and is committed to operational excellence. You will ensure the community operates in accordance with applicable federal, state, and local regulations while promoting a culture of teamwork, accountability, compassion, and respect.

    Compensation: 
    • $48,000.00 - $55,000.00 annually, dependent on experience

    Key Responsibilities: 
    • Oversee and management of the CBRF.
    • Serve as a role model and mentor to all employees.
    • DHS CH 83 Regulations comprehension and compliance.
    • Responsible for residents, staff and building 24 hours.
    • Works under and reports directly to Vice President of Operations, Carriage Healthcare Companies, Inc.
    • Work closely with Care Coordinator for clinical resident outcomes.
    •  Resident referral assessments. Include Care Coordinator when appropriate.
    • Upon resident admission: assess, initiate and complete all admission paperwork, complete assessments, i.e., fall/fall interventions/wanderer/Braden scale/evacuation, review and verify medications, treatments, and ADL’s in E-Mar, implement initial ISP.
    • Monitor staff progress notes for timely and accurate documentation.

    Qualifications: 
    Required: 
    • Must be at least 21 years of age
    • Demonstrate leadership and supervisory skills
    • Must speak and understand English
    • Previous experience in healthcare, geriatrics, or a long-term care setting
    • Patience, tact, enthusiasm and positive attitude toward the elderly
    • High school diploma
    Preferred Qualifications:
    • Leadership/Management experience in a healthcare or business setting
    • Education/Certification in healthcare
    • Experience working as a Certified Nursing Assistant, Resident Care Assistant, or Licensed Practical Nurse
    • Qualifications under DHS Chapter 83.15 specifically (d)
