Certified Nurse Assistant Pm Shift

RYZE West is seeking compassionate and dependable Certified Nursing Assistants (CNAs) to join our nursing team on the PM shift. CNAs provide direct resident care, assist with activities of daily living, and help maintain a safe, comfortable, and supportive environment for residents in our skilled nursing facility.

Duties Responsibilities

• Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, and personal hygiene.
• Assist residents with mobility, transfers, positioning, and ambulation.
• Monitor residents' conditions and report changes to the nursing staff promptly.
• Measure and record vital signs as directed.
• Assist residents with meals, hydration, and feeding as needed.
• Answer call lights promptly and respond to resident needs.
• Maintain accurate documentation of care provided.
• Follow infection prevention procedures and maintain a clean, safe resident environment.
• Provide compassionate care while respecting residents' dignity, privacy, and individual preferences.
• Assist with admissions, transfers, and discharges as needed.
• Communicate effectively with nurses and other members of the care team.
• Follow facility policies, procedures, and applicable safety standards.
• Perform other duties as assigned.

Qualifications

• Current, active Illinois CNA certification required.
• Previous CNA experience preferred.
• Skilled nursing or long-term care experience preferred.
• Strong communication and interpersonal skills.
• Compassionate, dependable, and attentive to resident needs.
• Ability to work effectively as part of a team in a fast-paced environment.
• Ability to perform the physical duties required for resident care.
