OnDeck Solutions is partnering with a skilled nursing facility in Lebanon, OH, to fill an immediate need for a COTA. Key Responsibilities
Provide skilled occupational therapy services to residents per the plan of care established by the supervising OT
Complete timely, accurate documentation in compliance with PDPM, MDS, and payer requirements
Collaborate with the interdisciplinary care team (OT) to support resident goals
Monitor and report resident progress, adjusting treatment approaches as directed
Maintain productivity standards consistent with facility expectations
Ensure all care is delivered in accordance with state practice act and facility policy
Qualifications
Active, unencumbered COTA license in OH
Current CPR/BLS certification
Prior SNF or geriatric experience strongly preferred
Working knowledge of PDPM and Medicare Part A/B documentation standards
Reliable transportation to facility location
Ability to pass background check and drug screening
