What You’ll Do:

• Implement individualized plans of care and provide high-quality clinical services.

• Administer medications and treatments according to nursing standards and departmental policies.

• Monitor and document vital signs, intake and output, and changes in condition.

• Supervise, delegate, and educate CNAs, NAs, and HHAs to support quality care.

• Assist Sisters with daily living activities, including dressing, bathing, grooming, ambulation, and meals.

• Follow safety protocols, infection control practices, and end-of-life care procedures.

• Coordinate and accompany Sisters to medical appointments and therapies.

• Maintain accurate documentation and communicate updates to the Clinical Manager.

• Participate in staff orientation, training, and continuing education.

• Support Activities and Enrichment programming.

• Perform other duties as assigned.

What You Bring:

• Current Licensed Practical Nurse (LPN) license in the state of Ohio.

• Current Basic Life Support (BLS) certification.

• Minimum of 5 years of nursing experience, preferably in geriatric or dementia care.

• Experience supervising or delegating care to others.

• Proficiency with Microsoft Word, Excel, and Outlook.

Preferred Qualifications:

• Experience in healthcare or residential care settings.

• Strong organizational, leadership, and communication skills.

• Ability to work independently and collaboratively with a team.
