Entira Family Clinics is looking for a highly motivated, friendly, compassionate Registered Nurse looking to be part of a team dedicated to providing exceptional quality care to our patients.

Full time, Monday-Friday in office. No remote opportunity

The expected annual pay range for this position will be from $60,000 to $80,000, plus core and optional employee benefits. Entira Family Clinics considers various factors in determining actual compensation for this position. Actual compensation will be commensurate with demonstrable level of experience and training, pertinent education including licensure and certifications, and other relevant business or organizational needs.

JOB SUMMARY:

The Clinic Nurse provides direct and coordinated nursing care to patients in a primary care setting. This position works closely with physicians, advanced practice providers, medical assistants, and other members of the healthcare team to deliver safe, efficient, and patient-centered care.

The Clinic Nurse is responsible for patient assessment, clinical care, care coordination, patient education, medication and treatment support, and follow-up. This role will also be responsible for telephone call triage, including assessing patient symptoms and concerns, determining the appropriate level and urgency of care, providing nursing guidance within established protocols, and escalating concerns to the appropriate provider or emergency services when indicated.

The Clinic Nurse promotes quality outcomes, patient safety, excellent patient experiences, and continuity of care while maintaining compliance with applicable clinical standards, organizational policies, and regulatory requirements.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide professional nursing care to patients in accordance with established clinical standards, policies, procedures, and scope of practice.
  • Perform patient assessments, obtain health histories, document vital signs and clinical information, and identify changes in patient condition.
  • Assist providers with examinations, procedures, treatments, and other clinical interventions.
  • Perform nursing procedures within scope of practice, including injections, immunizations, medication administration, wound care, specimen collection, and other treatments as applicable.
  • Perform telephone call triage by assessing patient symptoms, determining the urgency and appropriate disposition of the call, providing appropriate nursing advice within established protocols, and escalating concerns to the provider or emergency services when necessary.
  • Identify urgent and emergent symptoms during telephone and in-person interactions and ensure timely escalation and follow-up.
  • Respond to patient questions regarding symptoms, medications, treatments, test results, appointments, and care plans.
  • Coordinate and communicate patient needs with physicians, advanced practice providers, medical assistants, specialists, pharmacies, and other members of the care team.
  • Review and follow up on laboratory, diagnostic, and other test results according to established protocols and provider direction.
  • Communicate test results and provider recommendations to patients in accordance with established workflows.
  • Assist with medication management, including medication reconciliation, refill requests, patient education, and communication with providers and pharmacies.
  • Provide patient education regarding medications, disease management, preventive care, lifestyle modifications, and treatment plans.
  • Support chronic disease management and care coordination for patients with conditions such as diabetes, hypertension, asthma, heart disease, and other chronic illnesses.
  • Assist with preventive care activities, including immunizations, screenings, health maintenance, and wellness initiatives.
  • Monitor patient response to treatment and report significant findings or changes in condition to the appropriate provider.
  • Maintain accurate, timely, and thorough documentation in the electronic health record (EHR).
  • Follow established protocols for patient follow-up, referrals, care coordination, and transitions of care.
  • Assist with maintaining efficient patient flow and support clinic operations as needed.
  • Maintain appropriate infection prevention and control practices and follow patient safety procedures.
  • Protect patient confidentiality and comply with HIPAA and other applicable privacy and regulatory requirements.
  • Participate in quality improvement initiatives, audits, clinical projects, and other activities designed to improve patient care and clinic performance.
  • Maintain current knowledge of clinical practices, primary care standards, medications, and applicable policies and procedures.
  • Participate in staff meetings, training, education, and professional development activities.
  • Provide compassionate, respectful, and culturally responsive care to patients and families.
  • Collaborate effectively with the entire care team to promote continuity, safety, and quality of care.
  • Perform other duties as assigned.

EDUCATION AND EXPERIENCE:

Preferred

  • Bachelor of Science in Nursing (BSN).
  • Preferred experience in a clinic setting, Care Management and/or Triage
  • Experience with telephone nurse triage and use of standardized triage protocols.
  • Experience in primary care, family medicine, internal medicine, or a multi-specialty outpatient clinic.
  • Experience with chronic disease management and care coordination.
  • Experience with quality improvement and population health initiatives.
  • Familiarity with EHR systems and patient portal communication.
  • Certification in ambulatory care nursing or another relevant nursing specialty.
  • Experience working in a team-based primary care or patient-centered medical home environment.

QUALIFICATIONS AND SKILLS:

  • Demonstrate leadership qualities to guide, motivate and oversee team members
  • Ability to apply clinical knowledge and nursing judgment and providing and or coordinating community and clinic based services.
  • Ability to serve as an advocate for the patient
  • Ability to work effectively with individuals and families with multiple problems and needs.
  • Demonstrate computer skills and be able to access and input information within the Entira system.
