The Impact You'll Make
Holistic Support & Counseling: Use motivational interviewing and a whole-person approach to assess social, emotional, and financial factors, helping patients build actionable health goals.
Resource Navigation: Connect patients with essential community resources, including sliding fee scales, low-cost medications, transportation support, and post-hospital discharge care.
Brief Interventions: Provide brief individual counseling and utilize evidence-based tools (like CALM or CAMS models) to support patients navigating acute distress or barriers to external clinical care.
Care Coordination: Serve as the bridge between individuals, families, and multidisciplinary providers to ensure seamless communication and continuity of care.
Quality & Growth: Participate in patient-centered medical home (PCMH) initiatives, team huddles, and quality improvement projects (like PDSA cycles) to continuously enhance our patient care standards.
Who You Are
Licensed & Qualified: You hold an active professional license or certification aligned with Vermont standards (e.g., mental health counseling, social work, or addictions) and follow best practices outlined by your respective governing board.
