Core Responsibilities
  • Coordinate and oversee the scheduling and delivery of skilled nursing and therapy services for home health patients
  • Review and process physician orders, referrals, and authorizations in a timely and accurate manner
  • Conduct patient assessments and assist with the development of individualized care plans
  • Communicate regularly with field clinicians, physicians, patients, and family members to ensure care continuity
  • Monitor patient progress and update care plans to reflect changes in clinical condition
  • Ensure all clinical documentation meets regulatory and compliance standards, including Medicare and Medicaid guidelines
  • Participate in quality assurance and performance improvement initiatives
  • Mentor and support field nursing staff with clinical questions and documentation requirements
  • Collaborate with the billing and administrative teams to resolve authorization and documentation issues
  • Attend and contribute to case conference meetings and multidisciplinary team discussions

Must-Have Requirements
  • Active Registered Nurse (RN) license in the state of Texas, in good standing
  • Minimum of two years of clinical nursing experience, preferably in home health or a related setting
  • Strong understanding of Medicare Conditions of Participation and home health regulations
  • Proficiency with electronic medical record (EMR) systems
  • Excellent organizational, communication, and time management skills
  • Ability to manage multiple priorities simultaneously in a deadline-driven environment
  • Strong attention to detail and commitment to documentation accuracy
  • Bilingual skills in English and Spanish are a plus but not required
