Clinical Intake Coordinator – Home Care - Clearlake, CA - Weekday Day Shift

Use your healthcare knowledge and coordination skills to help patients access home health services. This role manages incoming referrals and keeps patients, clinicians, and referral sources informed.

Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m., 5×8-hour shifts.
Contract: November 9, 2026 – February 7, 2027

Key Responsibilities

• Process and document home care referrals.

• Coordinate referrals and admissions with patients, clinicians, physician offices, and community liaisons.

• Review patient needs and help ensure referrals reach the appropriate clinical team.

• Support intake calls, insurance verification, and communication with patients and case managers.

Requirements

• Associate degree in nursing or equivalent education/experience required.

• Current RN, LVN, or LPN license in the state of practice.

• EPIC experience required.

• At least 1.5 years of experience requested; clinic, home care, SNF, or hospital experience preferred.
