Pay Range: $38 - $40/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:- Evaluate member needs and available resources, recommending plans for optimal outcomes.
- Develop and monitor long-term care/service plans, identifying necessary providers and community resources.
- Facilitate communication between members, families, and care provider teams to ensure accessibility to services.
- Provide resource support for various needs, including employment, housing, and independent living.
- Conduct ongoing assessments and document care management activities to ensure compliance with regulations.
- Perform home and site visits to assess member needs and collaborate with healthcare providers.
- Educate members and families on healthcare procedures, service options, and benefits.
- Offer feedback to leadership on opportunities to enhance care quality and service delivery.
Key Requirements and Technology Experience:- Bachelor's degree in a human services field (psychology, social work, nursing, etc.) or equivalent experience.
- 2 – 4 years of related experience in care management or service delivery.
- RN or LCSW license required for specific states.
- Experience in LTSS and/or HCBS coordination and care management is preferred.
- Experience in social work, geriatrics, gerontology, pediatrics, or human services.
- LCSW-A preferred for North Carolina Tailored Plan.
