  • The <\/span>Clinical Manager<\/b> will <\/span>coordinate and oversee all direct care patient services provided by clinical personnel.
    <\/li>
  • You will conduct assessment and reassessment of patients, update plan of care, evaluate patients' needs and adhere to LeeKO's policies and procedures.
    <\/li>
  • Monitor and coordinate caregivers' assignments.
    <\/li>
  • Review referrals for acceptance ensuring patients meet admission criteria
    <\/li>
  • Admit patients and assigns appropriate clinicians based on the patient's needs<\/span><\/span><\/span><\/span>
    <\/li>
  • Effectively encourages high performance of service delivery and by being available to clinicians during business hours<\/span><\/span><\/span><\/span><\/span><\/span><\/span><\/span>
    <\/li>
  • Ensure documentation is in compliance with regulatory agencies and requirements of third -party payers
    <\/li>
  • Ensures final audits/billing are completed timely and in compliance with Medicaid/Medicare and third party payers regulations
    <\/li>
  • Coordinates communication between team members/attending physicians/caregivers to ensure the appropriateness of care and outcome planning
    <\/li>
  • Conduct interview and assist with on -boarding of newly hired clinicians
    <\/li>
  • Conduct annual performance evaluation
    <\/li>
  • Conduct QAPI and annual CEMP review
    <\/li>
  • Performs other related duties as requested by upper management

    Requirements<\/h3>


    <\/span><\/span><\/span><\/span><\/p>

    • Graduate of an accredited School of Nursing
      <\/li>
    • Current state or multi -state license as a Registered Nurse
      <\/li>
    • Current CPR.
      <\/li>
    • Valid driver’s license, with reliable transportation.
      <\/li>
    • Two years as a Registered Nurse in a home care
      <\/li>
    • Preferred at least one year of home health management experience
      <\/li><\/ul>
