Clinical Nurse Manager

• The Clinical Manager will coordinate and oversee all direct care patient services provided by clinical personnel.
  
• You will conduct assessment and reassessment of patients, update plan of care, evaluate patients' needs and adhere to LeeKO's policies and procedures.
  
• Monitor and coordinate caregivers' assignments.
  
• Review referrals for acceptance ensuring patients meet admission criteria
  
• Admit patients and assigns appropriate clinicians based on the patient's needs
  
• Effectively encourages high performance of service delivery and by being available to clinicians during business hours
  
• Ensure documentation is in compliance with regulatory agencies and requirements of third-party payers
  
• Ensures final audits/billing are completed timely and in compliance with Medicaid/Medicare and third party payers regulations
  
• Coordinates communication between team members/attending physicians/caregivers to ensure the appropriateness of care and outcome planning
  
• Conduct interview and assist with on-boarding of newly hired clinicians
  
• Conduct annual performance evaluation
  
• Conduct QAPI and annual CEMP review
  
• Performs other related duties as requested by upper management
  

Requirements

• Graduate of an accredited School of Nursing
  
• Current state or multi-state license as a Registered Nurse
  
• Current CPR.
  
• Valid driver’s license, with reliable transportation.
  
• Two years as a Registered Nurse in a home care
  
• Preferred at least one year of home health management experience
