    Qualifications:

    • A valid certification or degree as a licensed practical nurse (LPN) or licensed social worker (LSW).
    • An active and unrestricted license in the relevant state for this position.
    • Three-plus years of experience in hospital or post-acute care settings.
    • Proficiency in technology, including electronic medical systems and Microsoft Office.
    • The position requires a passion for improving patient outcomes and working in a collaborative team environment.
    • Authorized to work in the United States.

    Scheduling:

    • Enjoy flexible scheduling in a fast-paced environment with a Monday through Friday workweek from 7:00 AM to 4:00 PM (including a lunch break).
    • No nights, no weekends, and paid holidays.

    Key Responsibilities:

    • Partner with other Sound nurses, site leaders, and the hospitalist team to improve the quality and efficiency of hospitalized patient care.
    • Report to nursing leadership.
    • Improve patient outcomes by promoting health, preventing readmission, and addressing barriers to healthcare access before discharge.
    • Work to improve the patient experience and optimize resource utilization.
    • No direct patient care.
    • Interact directly with patients at the bedside through patient satisfaction and discharge preparedness conversations.

    Begin your journey as a Clinical Performance Nurse by partnering closely with hospital medicine teams to elevate patient care, improve outcomes, and strengthen clinical workflows. In these roles, you build expertise in performance improvement, data interpretation, documentation support, and clinical education - helping clinicians stay aligned, supported, and able to deliver exceptional care. As you progress through roles such as CPN, CPN‑Advanced, and Ambassador CPN‑Advanced, you deepen your impact, take on broader site support, and help drive consistency and connection across teams.

    • Site Leadership :

    Grow into leadership roles where you guide and support clinical performance at the facility or local cluster level. As a Clinical Performance Lead or Clinical Performance Supervisor, you coach and mentor CP teams, collaborate with hospital leadership, and ensure the day‑to‑day execution of performance strategies. You play a critical role in aligning teams, reinforcing best practices, and helping drive improvements in quality, documentation, throughput, and communication - all while fostering strong relationships with clinicians and hospital partners.

    • Specialty Leadership :

    Advance into wider-reaching leadership roles that shape performance across multiple hospitals, regions, and service lines. Leaders such as Managers and Directors of Clinical Performance bring together teams, processes, and analytics to support sustained performance and operational excellence. At the most senior levels - including Vice President of Clinical Performance and beyond - you help set organizational strategy, lead high‑impact initiatives, and influence national standards. These roles elevate the voice of nursing and ensure that clinical performance remains connected, collaborative, and consistently improving across Sound.
