Job Title: Clinical Pharmacist I
Location: Lake Charles, LA 70601
Duration: 13 weeks
Shift/Schedule: Full-time | Multiple day/evening shifts | 8, 10, or 12-hour shifts | Available shifts: 7:00 AM–3:30 PM, 8:00 AM–4:30 PM, 1:00 PM–9:00 PM, 2:00 PM–10:00 PM, 7:00 AM–5:00 PM, 11:00 AM–9:00 PM, 12:00 PM–10:00 PM, 7:00 AM–7:00 PM, or 8:00 AM–8:00 PM | Rotating weekends | No call | No floating
Job Responsibilities:
- Prepare and dispense medications according to physician orders, policies, procedures, and protocols.
- Interpret and verify verbal and written medication orders and maintain accurate patient medication records.
- Compound and dispense non-sterile and sterile products, chemotherapy, and parenteral nutrition products.
- Issue controlled substances and maintain required records.
- Monitor patient drug therapies for safety, effectiveness, contraindications, interactions, allergies, and appropriate dosing.
- Assist with pharmacokinetic consultations and renal drug dosing.
- Detect and report suspected adverse drug reactions.
- Provide clinical consultation and drug information to healthcare professionals.
- Provide medication education to patients and families.
- Participate in antibiotic stewardship, opioid stewardship, medication-use reviews, quality improvement, and shortage management.
- Participate in interdisciplinary rounds when assigned.
- Supervise pharmacy support personnel and verify pharmacy technician activities.
- Maintain the hospital drug formulary and promote appropriate, cost-effective medication therapy.
- Perform clinical interventions including IV-to-PO conversion, renal dosing, pharmacokinetic dosing, and anticoagulation management.
- Maintain current licensure, competency assessments, continuing education records, and departmental documentation.
- Provide pharmacy services efficiently, accurately, and in a timely manner.
Requirements:
- Doctor of Pharmacy (PharmD) or BS Pharmacy degree.
- Current Louisiana pharmacist license required.
- BLS certification required via American Heart Association.
- Hospital pharmacy experience preferred.
- Ability to provide safe and appropriate medication therapy in a hospital setting.
- Experience with ICU, cardiac cath lab, emergency department, behavioral health, and orthopedic surgery environments is a plus.
Preferences:
- Hospital pharmacy experience.
- ACLS certification via American Heart Association.
- Experience treating patients in a 17-bed ICU.
- Experience with cardiac cath lab, emergency department, inpatient behavioral health, and orthopedic surgery.
