Job Summary

Provides case management through CLTS Support and Service Coordination to children who are diagnosed with physical, emotional, or cognitive disabilities. The goal of the CLTS Waiver Program is to support children with substantial needs, as well as their parents/guardians, by delivering services to promote the goal of every child living their best possible life now and into the future.

  • This position may be eligible for a hybrid schedule to telework up to 3 days a week after successful completion of an initial employment period.

Essential Duties

Support and Service Coordination assists a child or youth and his or her family to achieve an inclusive, independent, and self-empowered life.

Support and Service Coordinators use their knowledge of available programs, resources, and services to create an individualized service plan that best supports the child and family in pursuit of their desired outcomes. This includes connecting families to supports throughout the community and promotes family members' self-determination and involvement in all facets of community life. At a minimum, monthly collateral contacts and direct contact with family every three months must occur.

Assisting to establish and maintain all aspects of program functional and financial eligibility. Provides participant and family education, support, and advocacy regarding their disability.

Assessing and periodically reassessing the child's or youth's health, safety, and functional capacity.

Reports incidents that create a significant risk or services harm to the physical, mental health, safety, or well-being of a child to the WI Department of Health Services.

Assures the application of client rights, grievance and dispute resolution processes and procedures.

Completes authorizations for purchase of service and obtains supervisory approval in line with service plans.

Monitors contracted services to assure high quality and the appropriate extent of services provided.

Interfaces with healthcare providers/insurers/Medical Assistance/Social Security to coordinate services.

Assists families with transition to adulthood process and connecting them to resources related to needs.

Identifies the need for and assists in the planning and development of new service resources as necessary.

Complies with applicable county, state and federal guidelines related to the provision of service.

Completes and maintains accurate and timely documentation according to applicable requirements, codes, contracts and policies.

Facilitates collaboration and maintains positive working relationships with and between participants, informal support systems, co-workers, health professionals, courts, legal representatives, contracted providers and the public at large.

Attends and contributes to staff meetings and overall unit performance.

Functions as part of treatment teams/multi-disciplinary teams to ensure continuity of care.

Minimum Qualifications Required

Education and Experience:

Bachelor's degree in social work or related human services field with a minimum of one year experience, within the last five years, working with persons with disabilities.

Licenses and Certifications:

Valid Wisconsin Drivers License

State Social Worker Certification preferred

Newly hired staff must complete and pass DHS approved Waiver Basics online training course and Mandated Reporter Online Training as well as becoming a certified Wisconsin Children's Functional Screener.

Knowledge, Skills & Abilities

Knowledge of case management principles and methods, family dynamics, human development, and behavior.

Knowledge of dynamics involved in community-based care for persons with long-term care

needs.

Knowledge of social work assessment principles and techniques.

Knowledge of client rights, grievance and dispute resolution processes and procedures.

Knowledge of state and federal laws, codes, procedures, and policies pertaining to Medicaid and Wisconsin Long-Term Care program delivery.

Knowledge of community resources.

Knowledge of needs and dynamics of those with all types of disabilities.

Ability to establish priorities for service coordination and to consider alternative treatment plans.

Ability to work independently.

Ability to use receive and provide feedback.

Ability to be decisive and to make sound decisions under the pressure of urgent situations.

Ability to plan and organize work to meet deadlines and to make effective use of time.

Ability to establish and maintain positive relationships with consumers and their family members, staff, contracted providers, and the public.

Ability to communicate effectively both orally and in writing.

Ability to work the required hours of the position, which is 37.5 hours weekly, with core hours being 8:00am-4:30pm.

PHYSICAL DEMANDS:

Lifting 20 pounds maximum with frequent lifting and/or carrying of objects weighing up to 10 pounds.

Intermittent standing, walking, and sitting; occasional driving.

Field work requiring face to face and collateral contacts located in home and community-based settings.

Using hand(s)/feet for repetitive single grasping, fine manipulation, pushing and pulling, and operating controls.

Occasional bending, twisting, squatting, climbing, reaching, and grappling.

Distinguishing sounds at various frequencies and volumes.

Distinguishing people or objects at varied distances under a variety of light conditions.
