• Assisted residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and feeding.
• Monitored and documented vital signs, weights, intake and output, and changes in resident condition.
• Provided compassionate, resident-centered care while promoting dignity, comfort, and independence.
• Assisted residents with ambulation, transfers, positioning, and range-of-motion exercises using proper body mechanics and lifting equipment.
• Responded promptly to call lights and resident requests to ensure safety and well-being.
• Observed and reported physical, emotional, and behavioral changes to licensed nursing staff.
• Assisted with meal service, hydration programs, and nutritional support.
• Maintained clean, safe, and organized resident rooms and care areas.
• Followed infection-control procedures, safety regulations, and facility policies.
• Collaborated with nurses, therapists, physicians, and interdisciplinary care teams to support quality resident care.
• Documented care provided in accordance with New Jersey CNA standards and facility requirements.
