Community Coordinator - Field (Brooklyn)

A nonprofit organization in Brooklyn, New York is seeking a Community Coordinator to support clients with housing, wellness, and care coordination services. This role focuses on connecting clients to providers, facilitating telehealth and field visits, and helping with day-to-day service needs in a community-based setting.

About the Opportunity:

• Shift: Day shift
• Schedule: Monday through Friday
• Hours: 9:00 AM - 5:00 PM
• Setting: Field work 90% (primarily field-based with some in-person office time)

Responsibilities:

• Coordinate client support services by connecting individuals with providers and community resources.
• Facilitate telehealth visits and assist clients with learning how to use telehealth platforms.
• Conduct field and home visits, including community, shelter, and home settings, as needed.
• Support appointment scheduling, transportation coordination, reminders, and client escort services.
• Track referrals, document client interactions, and complete required case notes and follow-up.

Qualifications:

• Bachelor's degree in a related field.
• Ability to perform regular field work and conduct client visits throughout the Brooklyn area.
• Experience working in a fast-paced, client-facing environment.

Desired Skills:

• Experience in mental health, social work, or community-based services.
• Proficiency with telehealth platforms, Zoom, and Microsoft Office.
