Job Description
A community services organization is seeking a Community Coordinator in Harlem, New York, to support clients with care coordination, telehealth facilitation, and field-based outreach. This role focuses on connecting individuals to providers, appointments, and community resources while helping promote stability and access to services.
About the Opportunity:
- Shift: Day shift
- Schedule: Monday-Friday
- Hours: 9:00 AM - 5:00 PM
- Setting: Hybrid (primarily field-based and in-office, with some remote work)
Responsibilities:
- Coordinate client care by connecting individuals to providers, appointments, and community-based resources
- Facilitate telehealth visits and support clients in using virtual care tools
- Conduct field visits in homes, shelters, and community settings as needed
- Support appointment scheduling, transportation coordination, and visit reminders
- Track client services, follow up on referrals, and complete documentation and notes
Qualifications:
- Bachelor's degree in a related field such as mental health, social work, or a similar discipline
- Experience in the mental health service field
Desired Skills:
- Experience working in case management or community outreach
- Proficiency with MS Office and database software
- Strong organizational, time management, and communication skills
- Compassionate, person-centered approach to client services
- Ability to work collaboratively as part of an interdisciplinary team
