The Prince Georges County Department of Family Services is currently seeking qualified applicants to fill a Community Developer 1G position grade G18 within its Aging & Disabilities Services Division.
About the Position
This is a senior level professional position which performs work in the support of the Retired Senior Volunteer Program (RSVP) for individuals aged 55 years and older who have gained a lifetime of experience and put their skills and talent to work in their communities. The incumbent is responsible for maintaining the RSVP program materials (written and electronic) to ensure that information is current and easily retrievable; prepares written and oral presentation describing results of analyses of research and planning information relative to the RSVP; works closely with host sites to ensure success of the Program; provides research and reports writing in the development of the comprehensive reports; and completes a variety of statistical and program reports necessary to ensure the continuation of local, state, or federal assistance, where provided. Incumbents may perform the more complex community development assignments subjects to extensive public contact.

The Department of Family Services improves the quality of life and overall well-being of the communities by providing information, assistance and referrals, as well as promoting and developing high quality, innovative programs that educate, empower, respect choice and preserve dignity. The department is responsible for providing assistance to some of the County's most vulnerable citizens - children, families, victims of domestic violence, people with intellectual and developmental disabilities, veterans, and seniors. The department is comprised of three divisions that serve the aging and disabled, victims of domestic violence, as well as children, youth and families in need of support and resources.
Examples of Work
Provide professional social/work/case management services to individuals seeking enrollment in one of the Medicaid Home and Community Based Programs (Community Options, Community First Choice, Community Personal Assistance Services, Money Follows the Person).
MFP Options Counselors will provide program education to individuals on the Medicaid programs eligibility criteria, and application/enrollment process.
Provide options counseling to nursing home residents and their guardians/family members interested in learning about services and support in the community specifically related to Medicaid programs. Assist gathering of financial documents and submission of CO Waiver applications.
Provide ongoing case management services to include evaluation of the individuals' need/medical assessment, assistance with development of care plans utilizing a person-centered approach.
Conducting quarterly visits to monitor and ensure that the services are being provided in accordance with the plan of service, program policies and procedures.
Make appropriate referrals for community resources based on client need, identifying Waiver and non-Waiver services.
Provide assistance to nursing home residents interested in transitioning to a community setting.
Assist with identifying affordable housing options and addressing housing related barriers.
Establish and maintain participants information and case records in the Program database and ensure that all information is current and updated.
Preparing case notes and other reports are required.
Collaborate with local Health Department, Dept of Social Services, Maryland Dept of Health and other community organizations concerning the operation of the Program and its participants.
Ability to take initiative, use sound judgement, exercise independent decision-making, and provide excellent customer service.

General Plan Information:
The Prince George's County benefits plan year is from January 1 to December 31.
A spouse (to include a same sex spouse) can be added to the health benefit plans. A marriage certificate and social security number is required to add a spouse.
Children under the age of 26 are eligible for coverage under the health benefit plans. This includes stepchildren and children of the same-sex spouse. A birth certificate(s) and social security number(s) is required to add a child(ren). If you are only adding the stepchildren or child(ren) of a same-sex spouse, you will need to submit a marriage certificate. You will also need to submit the birth certificate of the child(ren) and your spouse must be listed as a parent.
The premiums for health benefits are deducted on a pre-tax basis with the exception of Long-Term Disability, Extra Life Insurance and Voluntary Benefits (Short-Term Disability, Whole Life Insurance, Critical Illness, Accident Insurance, Cancer Indemnity, Hospital Indemnity Protection, Accident Indemnity Plan, Supplemental Dental and Group Legal Services).
New employees must enroll in the County's health benefit plans within thirty (30) days of the hire date.
The effective date of the health benefits coverage is the beginning of the month following a waiting period of forty-five (45) days from the date of hire.
After enrolling in the County's benefit plans, employees may only make changes to the plans either during the open enrollment period, which occurs annually (usually each October), and/or during the year, due to a family status change (i.e., marriage, births, divorce and adoption).
