    My AmeriCorps - Home Page Community Health Partners

    The Community Health Partners AmeriCorps Program, sponsored by The Health Foundation of Greater Indianapolis, is a statewide public health service initiative that strengthens Indiana's capacity to prevent, diagnose, treat, and respond to HIV, Hepatitis C (HCV), substance use disorder, and other emerging public health priorities. Members serve full-time with community-based organizations across Indiana, expanding access to outreach, education, referrals, health navigation, volunteer engagement, and organizational capacity-building services for communities most affected by health inequities and stigmatized conditions. Through structured service, professional development, and hands-on public health experience, the program prepares AmeriCorps Members for careers in public health, social work, health care, recovery support, and community leadership. The program also supports statewide efforts aligned with Indiana's Ending the HIV Epidemic strategy and Zero Is Possible plan by developing a trained, community-based workforce that can respond to local needs, reduce health disparities in urban/rural areas, and strengthen public health infrastructure.     Member Duties : AmeriCorps members will serve in high-impact community and clinical settings to expand HIV, STI, HCV, and substance use-related outreach and linkage services. At community-based sites, members will support confidential testing events, prevention education, PrEP/PEP referrals, health education, volunteer engagement, resource navigation, and follow-up for people living with or at risk for HIV. In emergency and clinical settings, members will help identify patients who may benefit from HCV testing, overdose prevention, recovery support, and linkage to care. Members will reduce barriers by connecting clients to case management, medical care, behavioral health, housing, food, transportation, and other social supports while strengthening host-site capacity through data collection, outreach coordination, and community engagement.
