Job Announcement: CHW Program Coordinator

ABOUT THE ROLE 

As a CHW Program Coordinator, you are the operational engine behind our CHW program in Georgia. You connect eligible patients to life-changing care, keep data clean and actionable, and ensure the program runs smoothly — even when things get complicated. You work closely with CHWs, Field Managers, and clinical partners, bridging the gap between people and the systems that support them.

WHO YOU ARE

You love data and people in equal measure. You have worked in roles like research assistant or program coordinator, where you enroll people into programs while also running and analyzing reports. You know how to do both — and do both well.

You are highly organized and can manage complex, multi-track workloads without losing threads. You excel at keeping the trains running and keeping track of multiple moving pieces. You work independently without constant direction — and when something breaks, you don't just fix the symptom. You fix the system.

Communication is one of your genuine strengths. You connect easily with patients and colleagues alike, bringing patience and open-mindedness to every interaction — whether you're reaching a hesitant patient on the phone or delivering a report to a clinical partner.

WHAT YOU'LL DO

Enroll patients & support the team

• Identify, contact, and enroll patients eligible for the IMPaCT CHW program

• Conduct persistent, multi-channel outreach to engage hard-to-reach patients

• Support CHW staff schedules and day-to-day program activities

• Track enrollment pipeline and flag gaps or delays proactively

Collect & analyze program data

• Collect and organize program data accurately and consistently according to protocol

• Build and run reports in Excel and program platforms to monitor enrollment, quality, and outcomes

• Translate data into actionable insights for the team and program leadership

• Support program evaluation to measure and communicate impact

Keep the program running well

• Manage multiple concurrent workstreams; keep timelines and deliverables on track

• Identify operational bottlenecks and recommend system-level improvements — not just quick fixes

• Use feedback and performance data as tools for continuous program improvement

WHAT YOU BRING

Required

• 1–3 years of experience in program coordination, research administration, or equivalent

• Demonstrated experience with data collection, analysis, and protocol implementation

• Proficiency in Microsoft Excel (reporting, data organization, pivot tables)

• Strong written and verbal communication skills

• Excellent organizational skills and keen attention to detail

• Ability to work independently and solve problems effectively

• Flexibility and adaptability in a fast-paced environment

• Ability to work on-site in Atlanta, GA

Preferred

• Bachelor's Degree in Public Health, Social Work, Healthcare Administration, or a related field

• Experience with an EMR system such as EPIC or case management platforms

• Familiarity with SDOH screening tools (e.g., Hunger Vital Sign, USDA Household Food Security)

• Experience with program or research enrollment workflows

• Knowledge of Medicaid, D-SNP, or public benefits programs

• Comfort working in community health, social services, or healthcare settings

WHAT TO EXPECT: WORK STYLE & SCHEDULE 

You will work closely with CHWs and Managers, and will have regular touchpoints with clinical partners and community organizations.

A typical week might include:

• Outreach calls and follow-ups to enroll or re-engage patients

• Running enrollment and program quality reports

• Supporting CHW scheduling and activity coordination

• Data entry, care plan updates, and documentation

• Team check-ins and coordination with clinical or partner staff

Schedule: Monday through Friday. Occasional flexibility may be needed to meet program needs.
