Job Summary
Responsible for working closely with individuals and communities to promote and maintain good health. Responsible for providing education, support, and outreach services to community members to help them achieve their health goals. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors. Works under the direction of a healthcare professional to provide preventive care and health education to members of the community.

High School Diploma required. Associate's Degree or Bachelor's Degree in related field of study preferred.
Valid Driver's License and reliable form of transportation required.
1+ years of experience in community outreach, health education or related field strongly preferred.

Schedule and Work Model:
Full-time, Monday through Friday, standard business hours.
Hybrid work model: Must be flexible to work in-office and/or travel in the community up to four days per week. Travel will be primarily within Acton, Concord, Maynard, Littleton, Westford and surrounding communities.
On remote workdays, employees must use a stable, secure, and compliant workstation in a quiet environment. Teams Video is required and must be accessed using MGB-provided equipment.
