The Opportunity
The Community Health Worker (CHW) is responsible providing support to patients and families as part of Rhode Island Primary Care interdisciplinary care team within a primary care setting. CHWs will be assigned to work with their assigned primary care and pediatric practices to coordinate care for a patient's social determinant of health (SDOH) needs. The RIPCPC Community health workers are an integral part of the care management team. In this role, assessment of patient bio-psychosocial strengths and challenges will be integral in supporting their healthcare needs and conditions. As a CHW you will coordinate with RIPCPC interdisciplinary care teams and community-based organizations to bridge the gap in services for our patients.

Physical Demands: Mostly sedentary work duties require exerting up to thirty pounds of force occasionally and/or small amounts of force frequently. Sedentary work typically involves sitting most of the time but may involve walking or standing for brief periods.
