Community Health Worker Full-Time About the Role Wayne Pediatrics, Inc is seeking a compassionate, community-rooted Community Health Worker (CHW) to join our team. In this role, you'll serve as a trusted bridge between patients, their families, and the healthcare system - helping people navigate care, access resources, and build the skills and support they need to manage their health. What You'll Do - Conduct home visits, phone check-ins, and in-clinic outreach to support patients in managing chronic conditions and care plans
- Help patients schedule and keep medical appointments, and accompany them when needed
- Connect patients with community resources (housing, food assistance, transportation, insurance enrollment, etc.)
- Provide basic health education on topics like chronic disease management, medication adherence, and preventive care
- Build trusting relationships with patients, especially those facing language, cultural, or socioeconomic barriers to care
- Document patient interactions and outcomes in the electronic health record (EHR)
- Collaborate with physicians, nurses, social workers, and care coordinators as part of an integrated care team
- Advocate for patients' needs within the healthcare system
- Participate in outreach events and community health initiatives
What You Bring - High school diploma or equivalent required; Associate's degree or CHW certification a plus
- Lived experience in or strong connection to the community served
- Excellent interpersonal and communication skills; ability to build rapport with diverse populations
- Comfort with basic computer use and EHR documentation (training provided)
- Valid driver's license and reliable transportation (if home visits are required)
- Ability to maintain confidentiality and work with sensitive patient information
- 1+ years of community outreach, healthcare, or social services experience preferred (not required for the right candidate)
