JOB SUMMARY
HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery.
The San Francisco Intake Services Program works with high-risk populations from a variety of backgrounds. Our programs are open and welcoming to adult participants, of any gender identity. We accept the majority of participants who seek Outpatient treatment, including walk-in's, referrals from other agencies and step-down transitional clients from residential treatment programs.
The Community Outreach Worker works in collaboration and continuous partnership with the healthcare teams to provide community outreach to link individuals to HealthRIGHT 360 for integrated primary care, behavioral health, substance use disorder treatment, oral healthcare, enabling resources and other services provided within the agency. This may include both in reach to other HealthRIGHT 360 programs as well outreach to single room occupancy hotels, navigation centers, community resources such as free dining rooms, and other areas that the target population may congregate. This position requires flexibility in hours that may include working evenings and weekends.

QUALIFICATIONS:
Education
Experience working with populations from various cultures and backgrounds and demonstrates ability to provide culturally competent support to patients.
Experience working with homeless, mentally ill, substance using, and criminal justice patients.
Experience working with populations with varying lifestyles, ages, sexual orientations, ethnic and cultural backgrounds, and economic status.
Experience working with populations suffering from addiction.
Experience working with mental health symptoms and disorders preferred.
Experience working with criminal justice population preferred.
