SUMMARY

The Tuberculosis (TB) Community Outreach Worker conducts both field and office work for the Tuberculosis Control Program. This position provides direct service by visiting TB patients in their homes to assess patient status, oversee medication administration and collect specimens. As a member of the TB Team, office work is also conducted to accomplish the goals of the Program.

This is considered an Essential Position within the Office of Public Health. Essential Employees are obligated to report to work as scheduled or remain at work during emergencies. Essential employees carry out essential job functions or assigned emergency duties which may involve assignments within the Department Operations Center, Emergency Operations Center, deployment to a field location to participate in public health activities, or other duties as required during periods of emergency. If received, emergency assignments may be performed under adverse conditions and continue for periods ranging from a few hours to several weeks, months, etc.

Reports directly to Public Health Nurse Supervisor, Tuberculosis Control Program Manager.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Conducting home visits to observe the ingestion of medication by TB patients.
• Assessing and reporting changes in the patients health status to the Public Health Nurse Case Manager.
• Collecting, packaging and shipping/delivering patient specimens.
• Assisting with correspondence to patients and providers.
• Maintaining program data and compiling reports.
• Engaging in educational and outreach efforts to high-risk populations to reduce TB in the community.
• Maintaining inventory of supplies and restocking clinic exam rooms.
• Other related duties, as required.
• Onsite in-person attendance is required as directed by Supervisor or Administrator.
• This position is subject to mandates requiring Child Abuse, Criminal History, and FBI Clearances for employment.

• High school diploma or equivalent required.

Experience and Skills

• Excellent inter-personal, written and oral communication skills.
• Excellent problem solving and decision-making skills.
• Knowledge of basic computer programs.
• Ability to work independently and travel throughout the County with reliable vehicle.
• Ability to collaborate with other organizations throughout the County.
• Ability to attend trainings and meetings in and out of State.
• Must be willing to work an occasional evening or weekend when required.
• Bilingual ability preferred.
