Community Resource Coordinator
Location: Remote/Field-Based – Candidate must reside in Albany, GA
Pay Rate: $25/hour
Schedule: Monday–Thursday and Friday, 9:00 AM–6:00 PM Monday–Thursday; Friday, 8:30 AM–5:30 PM
Job Summary
The Community Resource Coordinator supports health plan members by connecting them with community resources, healthcare services, and support programs. This role focuses on conducting home visits, identifying non-medical barriers to care, coordinating community outreach, and building partnerships with local organizations to improve member health outcomes.

- Conduct home visits with high-risk members to complete non-medical assessments and identify barriers to care, including home safety concerns, transportation challenges, and other social determinants of health.
- Connect members with appropriate community resources, healthcare services, and support programs.
- Build and maintain partnerships with community organizations and service providers.
- Coordinate, implement, and participate in community events throughout the designated region and potentially across Georgia.
- Conduct telephonic and in-person outreach to locate hard-to-reach individuals and families.
- Support nurses, social workers, and clinical teams through community education, outreach, informal guidance, and member support.
- Conduct non-clinical general health and non-medical assessments to identify member needs and facilitate appropriate referrals.
- Maintain accurate documentation of community resources and member interactions in accordance with health plan policies and standards.
- Assist members with community and care resource inquiries and provide educational materials to facilitate successful connections.

Qualifications
Qualifications
- High school diploma or equivalent required.
- 1–2 years of related experience required.
- Knowledge of community health practices, local resources, and social determinants of health.
- Strong communication, organizational, and time-management skills.
- Experience with data collection, reporting, and recordkeeping.
- Cultural competency and the ability to work with diverse populations.
- Reliable transportation and willingness to travel approximately 90% of the time throughout Georgia.
- Ability to work independently, adapt to changing community needs, and solve problems effectively.
