Pay Range: $29/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:- Connect members to known community and care resources to facilitate their care management journey.
- Support the coordination of community outreach resources and promote awareness of available services.
- Assist members with inquiries regarding community and care resources.
- Document and maintain community resources to comply with health plan standards and policies.
- Collaborate with clinical teams, providing outreach, community education, and member support.
- Conduct non-clinical health assessments to refer members to appropriate services.
- Perform telephonic and in-person outreach to engage hard-to-reach individuals and families.
- Visit homes and community organizations as needed.
- Understand and address Social Determinants of Health (SDOH) barriers.
- Comply with all policies and standards while performing other assigned duties.
Key Requirements and Technology Experience:- High School diploma or GED.
- 1 – 2 years of related experience.
