On-Site + Field-Based Position
Penn Home Health Care is seeking a dependable, organized, and hands-on Community Services Operations & Field Supervisor to support our Home Care and community-based services.
This is a hybrid position for someone who enjoys both office operations and working directly in the community. The employee will work from our Chester office when not performing field responsibilities and will travel to participant homes throughout Penn Home Health Care's service area for admissions, supervisory visits, field support, and temporary direct-care coverage when needed.
Penn Home Health Care serves participants across multiple counties. This position is not currently assigned to one specific county, so candidates must be comfortable traveling throughout multiple counties based on participant locations and operational needs.
As Penn Home Health Care grows, this position may also support additional community-based programs, including ODP services, after all applicable program-specific qualifications, clearances, training, and competency requirements have been completed.
Conduct assigned non-skilled Home Care admissions and case-opening visits when a licensed clinical assessment is not required.
Review authorized services and available intake information before the admission visit.
Complete required in-home admission activities.
Assist participants with admission documents and signatures when electronic completion is not feasible or the participant prefers to complete documents in person.
Complete applicable non-clinical home-safety and service information.
Prepare non-skilled Service Plans/Plans of Care based on authorized services, applicable assessment information, participant preferences, and identified needs.
Ensure completed admission documentation is submitted accurately and timely.
Recognize and escalate needs requiring nursing, clinical, Service Coordinator, payer, or management involvement.
Conduct assigned in-home supervisory and participant-monitoring visits in accordance with Agency and applicable program requirements.
Evaluate whether authorized services are being delivered according to the participant's Service Plan/Plan of Care.
Observe Direct Care Worker performance when required.
Review participant satisfaction, concerns, and changes in needs or circumstances.
Provide appropriate coaching and guidance to caregivers.
Document supervisory findings accurately and objectively.
Communicate concerns and follow-up needs to the appropriate Operations or management personnel.
Promptly escalate clinical concerns, safety issues, potential changes in service needs, suspected abuse, neglect or exploitation, and other matters requiring additional review.
Conduct assigned final in-office readiness appointments for newly cleared Direct Care Workers.
Issue Agency identification badges.
Confirm required onboarding steps have been completed according to the established clearance process.
Verify EVV/app readiness and assist with basic system readiness before assignment.
Provide applicable assignment information and communicate unresolved onboarding issues to the appropriate personnel.
Reinforce Agency expectations related to EVV, documentation, participant-specific service instructions, professional boundaries, and other non-clinical operational requirements.
Maintain the qualifications and demonstrated competencies required to provide authorized non-skilled direct-care services.
Provide temporary direct-care coverage when operationally necessary and appropriately assigned, including situations involving caregiver call-outs or newly opened cases awaiting permanent staffing.
Provide authorized personal assistance services according to the participant's Service Plan/Plan of Care and within demonstrated competency.
Help maintain continuity of services while Operations works to secure ongoing staffing.
Emergency and bridge coverage is an operational support responsibility and is not intended to replace the primary supervisory, admissions, office, and field responsibilities of this position.
When not performing field responsibilities, the Community Services Operations & Field Supervisor will maintain a physical presence in the office and support the Home Care Operations team as assigned.
Responsibilities may include:
Answering phones and assisting with office coverage.
Scheduling and service-gap resolution.
HHAeXchange support.
EVV monitoring and problem resolution.
Authorization-related workflow support.
Participant and caregiver communication.
Case coordination.
Operational monitoring.
Documentation follow-up.
Assisting with caregiver call-outs and replacement coverage.
Supporting other Home Care operational needs as assigned.
This position is cross-trained to support these functions but does not replace the Operations Specialists responsible for the day-to-day management of Home Care operations.
The Community Services Operations & Field Supervisor is responsible for timely and accurate documentation of assigned activities, including admissions, supervisory visits, participant monitoring, caregiver performance, Service Plans/Plans of Care, concerns, follow-up activities, and other required records.
The employee must communicate professionally with participants, families, caregivers, Operations staff, management, Service Coordinators, payers, and other appropriate parties while maintaining confidentiality and professional boundaries.
This is a non-licensed position.
The Community Services Operations & Field Supervisor does not independently perform nursing assessments, skilled nursing services, OASIS assessments, or other activities requiring a licensed clinician.
The employee must recognize when a situation exceeds their role or authority and promptly escalate clinical changes, safety concerns, authorization issues, potential Service Plan changes, incidents, suspected abuse/neglect/exploitation, or other matters requiring professional or management review.
High school diploma or equivalent required.
Experience in home care, personal assistance services, caregiving, field supervision, scheduling, case coordination, healthcare operations, or a related community-based service environment strongly preferred.
Current CNA certification preferred, but not required.
Must meet Penn Home Health Care's applicable Direct Care Worker qualification, clearance, training, health, and competency requirements before providing direct-care coverage.
Must be willing and able to provide hands-on non-skilled personal assistance when temporary or emergency coverage is required.
Comfortable learning and using electronic scheduling, EVV, documentation, and communication systems.
Strong organizational, communication, documentation, and problem-solving skills.
Ability to work independently and appropriately prioritize competing office and field responsibilities.
Ability to communicate professionally with participants, families, caregivers, and members of the Agency team.
Reliable transportation and the ability and willingness to travel throughout multiple counties within Penn Home Health Care's service area are required.
Penn Home Health Care serves participants across multiple counties. This position requires regular travel to participant homes throughout the Agency's service area for admissions, supervisory visits, field support, participant monitoring, and temporary service coverage.
Candidates must have reliable transportation and be willing and able to travel across multiple counties based on operational needs and participant locations.
This position is not currently assigned to a single county.
As Penn Home Health Care's census and field team grow, the Agency may transition to a more localized structure in which Community Services Operations & Field Supervisors are assigned primarily to specific counties or geographic service areas.
Mileage is not separately reimbursed.
As Penn Home Health Care expands its community-based programs, this position may support additional approved service lines, including ODP services, after completion of all applicable program-specific training, qualifications, competencies, and clearance requirements.
The responsibilities of this position may evolve as the Agency grows and additional Community Services Operations & Field Supervisors are added.
This position will primarily support Penn Home Health Care's normal office and field operations.
Specific work hours and field scheduling will be discussed during the hiring process. Flexibility is required because field assignments, participant admissions, supervisory visits, and unexpected service-coverage needs may occur during scheduled working hours.

This is an on-site and field-based position. When not assigned to field responsibilities, the employee will work from the Chester office. Field assignments may require travel throughout multiple counties within Penn Home Health Care's service area.
This is not a desk-only position, and it is not exclusively a caregiving position.
The successful candidate must be comfortable moving between office operations, participant homes, case openings, caregiver supervision, participant monitoring, field problem-solving, and temporary direct-care coverage based on the needs of the Agency.
The position is designed for someone who can help Penn Home Health Care keep services moving both in the office and in the field.
