Job Requirements

  • Education Requirements: Valid Licensed Practical Nurse (LPN) license.
  • Certifications Requirements: BLS certification; LTC/LTAC experience required; ACLS & Wound Care certifications preferred.
  • Experience Requirements: At least 1 year of LPN experience in Long-Term Acute Care (LTAC) or Long-Term Care (LTC) settings.
  • Other Requirements: Ability to work rotating weekend shifts; excellent communication skills; ability to adapt to rotating schedules; proficiency in electronic medical records; strong teamwork and reliability.

  • Provide compassionate and high-quality nursing care to LTC and LTAC patients, adhering to facility guidelines and protocols.
  • Collaborate effectively with interdisciplinary teams to develop and implement patient care plans.
  • Maintain accurate and thorough documentation of patient assessments, treatments, and progress in electronic medical records.
  • Administer medications and treatments in accordance with physician orders and facility policies.
  • Execute patient assessments, monitor vital signs, and respond promptly to patient needs or emergencies during rotating shifts.
