    The Montana Department of Public Health and Human Services (DPHHS) has a career opportunity within the Behavioral Health & Developmental Disabilities Division.

    The Developmental Disabilities Program procures and supports community-based services for people with developmental disabilities and/or their families.

    The DDP Targeted Case Manager is responsible for a comprehensive assessment and periodic reassessment of individual needs, the development of and periodic revision of the plan of care, referral and related activities to help the member obtain needed services, and monitoring and follow-up activities. The Case Manager is responsible to ensure the quality of the plan of care.

    Qualifications for this Career Opportunity

    • Above average ability to follow written and verbal directions

    • Ability to communicate both orally and in writing.

    • Above average ability to recognize, analyze, and solve complex problems/dilemmas.

    • Ability to facilitate, negotiate and mediate problems as well as promote team consensus.

    • Meet minimum qualifications:

    • Bachelor's degree in social work or a related field from an accredited college.

    • Minimum of two years of experience in human services, with at least one year of experience in the field of developmental disabilities (DD). If lacking one year of experience in field of DD, you must complete at least 40 hours of training in service delivery to persons with DD under a training plan reviewed by the Department within no more than three months of hire or designation as a case manager.

    Additional Information

    This position may be eligible for partial telework within the state of Montana. Specific conditions will be provided by the supervisor and must adhere to state policy.
